The field of performing transcatheter interventions to treat vascular lesions has exploded over the past 20 years. Gastrointestinal stomas constitute an evident anatomical modification, but their perception by patients represents a serious and unacceptable alteration of the body schema. Esophagostomy, gastrostomy, and jejunostomy are usually temporary, but ileostomy and colostomy are permanent in many cases. Two purposes characterize the two types of intestinal stomas: to divert intestinal content and to allow enteral nutrition. Despite being commonly prescribed in inpatient settings, intravenous fluids are often administered without evidence-based guidelines, neglecting essential considerations such as the fact they are drugs, that need proper dosing, duration, indications, contraindications, side effects, and de-escalation. This book fills this knowledge gap. Each chapter starts with a clinical vignette and a related question, followed by a concise summary abstract, key learning points, main text, and a conclusion that addresses the clinical question and provides relevant take-home messages. Additionally, every chapter includes an IFA commentary referencing the latest literature and evidence. The book begins with an introductory chapter that provides definitions and terminology, followed by four sections. The first section covers the fundamentals of intravenous fluid therapy, including fluid compartments in sick patients, fluid bolus dynamics, heart-lung interactions, fluid challenge, fluid responsiveness, arterial blood gas analysis based on traditional and Stewart concepts, and electrolyte physiology relevant to clinical issues. The second section discusses commercially available intravenous fluids, while the third section focuses on fluid therapy in specific clinical situations. The final section introduces the concepts of fluid stewardship and appropriate fluid prescription. The subspecialty of reconstructive microsurgery utilizes microsurgical techniques to allow surgeons to operate on the smallest of structures, including vessels, lymphatics, and nerves. Traditionally, these procedures have been used by microsurgeons to reconstruct complex tissue defects through the use of free tissue transfer, which involves the transfer of tissue (free flaps) between distant anatomic sites on the body. As the field has advanced, microsurgical techniques have been applied to even smaller structures, opening the door for new procedures, such as lymphatic reconstruction in the management of lymphedema and new types of free flaps. In addition, refinement in the technique has allowed microsurgeons to now perform many of these procedures with minimal donor site morbidity. Lastly, trends in plastic and reconstructive surgery have also resulted in the more widespread use of other applications for microsurgery, such as gender-affirming surgery and vascularized tissue allotransplantation. In this Special Issue, the latest research in reconstructive microsurgery is presented, including in the areas of microsurgical technique, robotic microsurgery, and program development.

Dramatic improvements in medicine over the last few years have resulted in more reliable and accessible diagnostics and treatment of rectal cancer. Colorectal cancer remains a major health issue for many developed regions around the world. The good news is that early detection has significantly improved overall survival rates and continues to do so. A number of prevention strategies contribute to this positive trend, and today a patient who undergoes a colonoscopy for screening purposes stands a much better chance of being effectively surveyed for prevention of colorectal cancer. Patients can rely increasingly on the improved datasets and technical advances that are being made in screening approaches and skills. With continued progress, particularly in the partnership between clinicians and computer scientists, the future for colorectal cancer surveillance looks increasingly positive for the development of improved tools and methods.

More than eighty percent of the worldwide deaths related to sepsis occur in resource-limited settings in low and middle-income countries. Current international sepsis guidelines cannot be implemented without adaptations towards these settings, mainly because of the difference in local resources and a different spectrum of infectious diseases causing sepsis. This prompted members of the Global Intensive Care working group of the European Society of Intensive Care Medicine (ESICM) and the Mahidol-Oxford Tropical Medicine Research Unit (MORU, Bangkok, Thailand) - among which the Editors – to develop with an international group of experts a comprehensive set of recommendations for the management of sepsis in resource-limited settings. Recommendations are based on both current scientific evidence and clinical experience of clinicians working in resource-limited settings. The book includes an overview chapter outlining the current challenges and future directions of sepsis management as well as general recommendations on the structure and organization of intensive care services in resource-limited settings. Specific recommendations on the recognition and management of patients with sepsis and septic shock in these settings are grouped into seven chapters. The book provides evidence-based practical guidance for doctors in low and middle income countries treating patients with sepsis, and highlights areas for further research and discussion.

The procedure of skin grafting has been performed since 3000BC and with the aid of modern technology has evolved through the years. While the development of new techniques and devices has significantly improved the functional as well as the aesthetic results from skin grafting, the fundamentals of skin grafting have remained the same, a healthy vascular granulating wound bed free of infection. Adherence to the recipient bed is the most important factor in skin graft survival and research continues introducing new techniques that promote this process. It may take a few days or a week to recover from a less complex operation, but it can take a few months to recover from major surgery. The American Society of Anesthesiologists Physical Status (ASA-PS) classification has long been used as a ranking system that quantifies patient health before anesthesia and surgery. It is widely used to determine a patient’s likelihood of developing postoperative complications. ERAS is the acronym of Enhanced Recovery After Surgery: a multimodal perioperative approach based on the best medical evidence. The aim of this program is to try to change the physiological and psychological responses to major surgery. Malnutrition is one of the most important patient-related factors affecting morbidity and mortality in surgical patients. In addition, infections are important in the recovery process after surgery.
